The 2nd Division of the Spinal Surgery Unit at the Istituto Ortopedico Galeazzi Research Hospital is a National Reference Centre of the Italian Society for Spinal Surgery. It is a training centre for surgeons specialised in Orthopaedics and Neurosurgery. It is also a centre for the Masters programme in Spinal Surgery of the Italian Society of Spine Surgery as well as a home to the international courses of AOSpine International.

The Department is a permanent training site for surgeons from all over Europe and Overseas countries for the refinement of minimally invasive fusion techniques (ALIF technique with “perinavel”, Lateral and Oblique approach).

The director of the Department is Dr. Roberto Bassani. He is a member of several faculty board as well an international lecturer, involved in many training courses for spinal surgeons. He is an active member of the Italian Society of Spine Surgery (GIS), and a member of the board of AOSpine and other high-level scientific societies such as the American Scoliosis Research Society (SRS) and EUROSPINE.

The main clinical and surgical areas are: congenital and degenerative pathologies of the spine and huge deformity.  The main research field is the study and development of anterior approaches to the spine using minimally invasive techniques based on the respect of the normal anatomy.

Traumatic or degenerative disc disease

Treatment of lumbar and cervical disc herniation (microdiscectomy). Simple cervical, thoracic and lumbar discectomy with microsurgical approach for disc herniations are offered (using magnification or endoscopic assistance). In addition, surgical treatment of degenerative (interbody fusion) or isthmic spondylolisthesis (isthmus reconstruction). Traditional and mini, video-assisted techniques are a point of excellence at the Unit. Surgical treatment of "chronic back pain" is offered using functional reconstructive techniques (i.e. lumbar Total disc replacement) or interbody fusion using a minimal invasive approach (original “perinavel” approach).
Surgical treatment is also available for symptomatic cervical discopathy, with or without myelopathy, by replacing the disk with a mobile device or fusion.

Treatment for cervical myelopathy by decompression and reconstruction of the cervical spine by anterior or posterior approach are a point of excellence of the Unit.

Last but not least, treatment of vertebral fractures due to osteoporosis or metastatic osteolytic lesions is done using traditional techniques by instrumented fusion or minimally invasive percutaneous instrumentation or interbody vertebroplasty or kyphoplasty.

Main Pathologies Treated:

  • Disc herniation (cervical, thoracic, lumbar)
  • Vertebral fractures
  • Degenerative or Isthmic spondylolisthesis (even High grade  spondylolisthesis)
  • Chronic low back pain

Top Procedures:

  • Minimally invasive approaches (endoscopic/thoracoscopic)
  • Total disc replacement (TDR)

Treatment of deformities

The surgical team is highly specialized in the correction and stabilization of scoliosis and spondylolisthesis (instrumented fusion) as well as correction of congenital and acquired deformities such as scoliosis, kyphosis using traditional techniques and minimally invasive techniques (combined anterior/lateral/posterior approaches).

Main Pathologies Treated:

  •  Adolescent scoliosis
  • De Novo (adult) scoliosis
  • Sagittal imbalance
  • Post traumatic deformity
  • Failed back syndrome
  • Revision of surgical complications:
    • Non-fusion
    • Proximal junctional kyphosis (PJK)
    • Post laminectomy instability
  • All cervical pathologies

Top Surgical Procedures:

  • Traditional methods: posterior instrumented fusion
  • Minimally invasive technique for anterior lumbar interbody fusion
    • “Perinavel” Approach (original published technique)

Mini Invasive Approach

Minimally Invasive Approach can be applied to a multitude of pathologies connected to spinal problems:  low back pain; persistent sciatica; spinal instability; vertebral fractures and so on. The Team is highly specialized in minimally invasive anterior techniques to the lumbar spine to remove degenerated disc to create proper lordosis and get interbody fusion.

Main Pathologies Treated:

  • Lower back pain
  • Persistent Sciatica
  • Spinal instability
  • Vertebral fractures

  • HD ENCOSCOPE for MIS anterior to the lumbar spine
  • Original MIS retractor for MIS anterior & posterior
  • 3D C ARM and O-ARM for complex deformity
  • Jackson table for huge deformity and multiple approaches
