Persistent postural perceptual dizziness (PPPD)
What is it?
Persistent postural-perceptual dizziness is a clinical condition characterised by an almost constant presence of unsteadiness for most of the day for at least three consecutive months. The onset of symptoms often follows acute vestibular pathology (VPPB, vestibular neuritis, etc.) and/or significant psychological experiences.
Which are the symptoms?
The main symptom is the presence of unsteadiness for most of the day for 3 consecutive months. The symptoms increase with active and/or passive body movements and tend to decrease when lying down.
- Persistent unsteadiness
How is it diagnosed?
Diagnosis is anamnestic and involves a careful collection of clinical and symptomatic anamnesis. Special questionnaires are used. A complete vestibular examination, which is important in the diagnostic process, is used to exclude other pathological conditions causing the symptoms.
- Collecting medical history
- Questionnaire
- Vestibular assessment
How is it treated?
To date, the main treatment includes a pharmacological approach using the selective serotonin reuptake inhibitors (SSRI) and mood stabilising drugs, in addition to vestibular rehabilitation and cognitive-behavioural therapy.
