Teleradiography of the spine (TeleRX)
What is it?
The primary examination is the teleradiography of the spine (Tele X-ray) in 2 projections in orthostasis and X-ray with dynamic projections in flexion and extension (supine). The X-ray can assess the presence and extent of vertebral slippage and any associated conditions (isthmic lysis, disc deneneration, fractures, infections, tumors). Other radiologic examinations such as MRI and CT may be performed as necessary to confirm and clarify the diagnosis.
When is this exam indicated?
Diagnostic radiological examinations are recommended in cases of low back pain refractory with conservative treatment and/or in the presence of associated radicular nerve disorders. MRI and CT are necessary to plan the type of treatment and essential in planning surgery. Spine teleradiography (Tele X-ray) in orthostasis with standard or low-dose-radiation (EOS) technique allows the assessment of the listhesis in anteroposterior and laterolateral projection. The acquisition of these two planes plays a major role, as it allows to study the sagittal and coronal balance, measure in quantitative terms the degrees of the main curves and design the preoperative planning. The proper friction reduction (when applicable) allows to minimize mechanical pain and the risk of junctional pathologies.
How is it performed?
The teleradiography of the spine (Tele X-ray) in orthostasis with standard technique is performed through the use of X-rays obtaining an image of the skeleton in two projections. The low-dose radiography of the spine allows to perform the same examination but with a significantly lower amount of radiation and with the possibility of performing three-dimensional reconstructions of the column or the entire skeleton.
Contraindications
Teleradiography of the spine (TeleRX) and CT are contraindicated for pregnant women. Major contraindications for MRI are: non-MRI compatible metals, metal fragments, dental prosthesis, pregnancy, pace-maker, intra-uterine devices (IUD). In case of use of contrast medium (both iodine for CT and gadolinium for MRI), rarely expected, the patient will have to follow a proper anti-allergic preparation.
